Reports from the LINCS Healthcare Database.

The following report is based on the LINCS Healthcare (LINCS) database and which is derived from a 12 month period - July 1, 2010 to June 30, 2011.

This report includes findings about the Type of Services that LINCS provides in terms of actual 'Number of Visits'; the 'Length of Stay' (LOS) of these visits and; 'Patient Outcomes' - with a high rate of satisfactory completion of cares and very low readmission to hospital rates (less than 5%).

Key findings show:

1. LINCS has provided approximately 8,000 Nursing and AIN cares. Of these cares, LINCS carried out 3,983 complex clinical nursing cares involving – 1,855 Wound Cares, 1,593 IV cares, and 535 Negative Pressure Dressing cares.
2. The mean length of stay(visits) for all types of cares is 7.6 visits
3. Approximately 53% of all of our patients were aged 71 years and over. The Age range is 15-99.
4. 82% of our patients successfully completed their stay with us (they either went back to regaining their normal daily activities (87%) or were discharged to a community care provider such as a HACC provider or a nursing home (13%).
5. We have a less than 5% readmission rate to hospital (3.6%).

Two of these findings are especially significant

• A high rate of successful completion of patient stay (82%) and
• A 3.6% hospital readmission rate

These findings demonstrate that LINCS not only provides the full range of cares, from General nursing and AIN cares to quite complex Clinical Nursing cares, but that we do so in a competent, timely, and efficient manner, and within funding parameters.
